Instructions: Prior to uploading the user's access forms, please ensure the proper naming convention of the file is being used. This includes the user's last name, user's first name, user's middle initial and the type of file. Please refrain from including commas in the file name. Here are a few examples for you:

  • Smith Jane L 2875

The training artifact should include the training name (IA or CA), the completion date and the agency.

Place all files (single user) into a folder, right-click the folder and select: Send
To > Compressed Folder
. This will create a single ".zip" file.

All forms have been completed digitally and are in compliance with the instructions located on each form. For new users, the proper Information Assurance Awareness or Cyber Awareness Challenge training artifact has been included. For existing uses, I've checked the user's access in the DPAS application to ensure the change being requested is new and not a duplication of the users existing access.
